Health Center Staff

The School-Based Health Center is a joint effort of Family Centers, Inc., The Stamford Health Department and the Stamford Public Schools.

All children enrolled in Dolan Middle School may use the School Based Health Center if they are signed up regardless of income or health care coverage.  Forms can be picked up in the School Based Health Center or in the Main Office.  These forms are to be completed by the parents and sent back to the Health Center.     The Health Center services include:

• Physical Examinations
• Treatment of Minor Episodic Illnesses & Injuries
• Preventive Health Education Programs
• Nutritional Guidelines
• Counseling for Mental Health Issues
