The Dolan/Toquam School Based Health Center (SBHC), located on the first floor of Dolan Middle School, is staffed by a Nurse Practitioner, Social Worker and a Program Assistant who work in collaboration with the school nurse, guidance counselors, school social workers, school psychologists, teachers, and other school staff to help ensure every child's academic success. SBHC staff combine health education, preventive services and disease management to positively impact students, encourage healthy behaviors and promote learning.

Dolan's School Nurse services all students currently enrolled in school.
HEALTH CENTER
Middle School can be a challenging time in the life of a young adolescent. Every
day we work to provide the services and support children need to build their
sense of self worth, succeed in school and help them grow into healthy adults.
SBHC services include:
• Physical Examinations
• Immunizations
• Nutritional Guidance
• Preventive Health Education Programs
• Treatment of Minor Illness and Injuries
• Referrals to Specialist and Medical follow up
• Assessment of Skin Problems
• Asthma education and follow up
• Individual and Group Counseling
• Laboratory Tests (Strep, TB, Anemia, etc.)
Services are provided to all SBHC-enrolled students regardless of their ability
to pay. You do not need
insurance to access services; however when coverage exists SBHC is required to
bill these insurance
companies. The SBHC will not bill parents/guardians or students for any
co-payments required by insurance; will not seek payment from you if you have
not met your insurance deductible; and will not seek direct payment from you if
the claim submitted for services is denied by the insurance company as these are
covered by existing grants.
The SBHC provides Dolan students convenient services to supplement care provided
in the community, all on-site at Dolan Middle School. Please look for the School
Based Health Centers Permission Form
(printed on blue paper this year), provide the necessary information, sign and
return to the Family Centers Inc., School Based Health Center at Dolan Middle
School.
Please remember, we must have a signed permission form on file
in order for your child to access this on-site service.
Dolan Schools Public
Health Dental Hygienist is Mrs. Baranowski; she can be reached at (203) 977-5519
and is known to your children as Mrs. B.
The Dental Health Program is a prevention-oriented program, provided by the
Department of
Health, Public Health Dental Hygienists who are educated licensed professionals.
There are
four parts this program.
Dental Health Education
The Dental Hygienist provides classroom education appropriate to grade level.
Education is also provided for parents, faculty and other school groups.
Dental Health Screening
The screening focuses attention on wellness and health promoting preventive
services.
If dental concerns are found, parents will be notified. This screening does not
take the
place of regular 6 month dental checkups.
Dental Sealant Program*
This program is offered at no cost to the citys public school second grade
students.
Dental sealants are thin, protective plastic coatings applied to fully erupted,
healthy,
permanent molars. Sealants fill the groves on the chewing surfaces of molars to
prevent
cavities.
Dental Services For Students
The Stamford Health Department operates six licensed Dental Clinics. The Public
Health Dental Hygienists offer the following preventive services:
Assessment *Sealants X-Rays
Teeth Cleaning Fluoride Treatments
By promoting preventives services, the program focuses attention on wellness and
health.
Dolan students can receive care at Toquam and KT Murphy Schools. If restorative
services
are needed, the dental hygienist will help you locate a dentist.
ALL SERVICES ARE FREE to Stamford School Students.
